How to Complete Patient Portal Forms on your Cell Phone

  2. Sign in with your ÁñÁ«ÊÓƵ Portal credentials.

  3. Click the menu bar (grey bar with three blue lines).

  4. Click on “Forms”.

  5. Scroll down to “ÁñÁ«ÊÓƵ Patient Forms” and complete the following forms: ÁñÁ«ÊÓƵ Medical History, ÁñÁ«ÊÓƵ Privacy Policy Acknowledgement, ÁñÁ«ÊÓƵ Release Authorization, and all other forms related to your visit (ÁñÁ«ÊÓƵ Physical Form for Physicals, STI Test Consent Form for Sexual Health/UTI Visits, TB Questionnaire for TB Skin Tests, Women’s Health Form for Women’s Health Visits, etc.)

  6. Go to “Messages” for Class Excuses. Go to “Statements” for receipts from your visit.
